It also allows our conversations to be documented in the medical record. If you do not have the app, you can instead email me. The app does not currently support sending documents so please use email to send me documents or photos if needed (send to Joanne for upload - [email protected]) but also message me in the app with questions or if you want a reply. This will help keep me organized and ensure your information does not get lost. There are three primary reasons you may want to contact the clinic: 1. Administrative Concerns 2. Medical Concerns 3. Urgent Concerns They each have a different protocol. ## Administrative Concerns Joanne is in charge of administrative tasks. This will include: - Writing notes (work notes, notes of medical necessity, etc.) - Filling out FMLA forms - Sending faxes - Prior authorizations - Scheduling appointments - Cancelling appointments - Uploading medical records If any of these need to be done, please email Joanne at [email protected]. She will forward notes and paperwork to me to sign after they have been filled out. ## Medical Concerns All medical concerns should now be communicated through the SigmaMD app. If you do not have the SigmaMD app, please schedule a brief or full length appointment with me to discuss. I can no longer address medical concerns via text or email. If the concern is urgent, contact me urgently for an immediate phone call or same day appointment. ## Urgent Concerns > [!warning] Urgent Communication > To contact me urgently, use my cell phone number: (503) 886-9516 > > For an IMMEDIATE response, please call twice in a row to break through do-not-disturb filters. For same day response, please text "URGENT." If you need to contact me urgently, please text "URGENT" to my cell phone at (503) 886-9516 followed by your name. I will call you. If I do not call you within 10 minutes please call me twice in a row to break through my "do not disturb" settings. This will wake me from sleep, interrupt the movie at the movie theater, have me hopping out of the shower with shampoo in my hair, and otherwise get my attention immediately unless it is off or out of earshot. Please use this power judiciously. I cannot guarantee an immediate response so please be ready to utilize convenient care or the emergency room as a backup if I don’t get back to you quickly. If you have tried to reach me through normal office communications and I have not responded, you may reach out to me via text on my cell phone to draw attention to the delay, especially if there is an approaching deadline or if there is an urgent need.
