# 20240121 ☕️ Sunday Morning Coffee Notes Three `random notes` from my Obsidian vault, today's `music` selection from MailTape, and a `photo` from the past week from my phone. This is how I am dipping my toe into [Obsidian Publish](https://publish.obsidian.md) by adding at least 3 notes per week from my library of 6800+ notes. --- ## 🎲 [[5HT-2A Antagonists]] (2013) This is an old list of serotonin (aka 5HT) 2a receptor antagonists and their Ki values. The Ki, or inhibitory constant, is the concentration of a drug at the site of action (in this case, in the brain) that causes a 50% reduction in the receptors activity. So, for #mirtazapine (Remeron), a concentration of only 10 nanomolar blocks half of the 5HT2a receptors. Because 5HT2a receptors tend to increase #anxiety when stimulated, #5HT2a receptor blockers (antagonists) will generally reduce anxiety. It is helpful to know the general range of Ki values for prescribed drugs at various receptors because it gives you an idea of how effective a medication may be at that receptor. There are other factors to consider, but this one helps to define how potent medications may be. --- ## 🎲 [[19430121.GothenburgTimes.Rowson|Lester butchers a cow]] (1943) Back in the day, newspapers carried news of small events, like who had dinner guest on Sunday or who came down with the flu. Not unlike today's social media. Exactly 81 years ago Lester, Eli, and Everett butchered a cow. (Now you know.) --- ## 🎲 [[OPASS-2.3.2.1]] (2019) Before I started the position of medical director for Maryland #Medicaid for Optum, I read through the entire #RFP (request for proposal) to understand what was involved. I broke down the RFP into its various section and hyperlinked them to make it easier to find stuff. This is one of those sections -- it happens to be about certain provider management activities.
